If you would like to become an approved retailer, please fill out the form below to be contacted with more information and the necessary forms to set up your account.

Wholesale Account Set Up Requirements:

• The order minimum is $500.00 per store.
• A copy of your Resale Certificate.
• A completed Wholesale Agreement Form.
• A completed Wholesale Account Info Form.
• Prepayment for your wholesale goods by CC or Bank Deposit upon approval, no terms accepted.

Once we have reviewed and approved your submission we will send you our catalog and pricing.
